Respite Care, Housing, and Transportation Services
Respite Care
Respite services are intended to provide the primary caregiver, and person receiving supports, a break from day-to-day care. This may be overnight, during a caregiver’s travel, or anytime the caregiver needs respite for daily care responsibilities. Respite is provided on an hourly basis, and may occur in the person’s own home, or the home of the Respite staff if preferred by the individual in services. People may request up to 720 hours or respite each year in the Person- Centered Plan.
Housing Support Services
Housing Support Services are time-limited supports to assist people in services to identify and navigate housing opportunities, address or overcome barriers to housing, and obtain housing or remain in the current rented or leased home.
This service includes housing information, assistance, transition, and tenancy sustaining services. The Housing Information and Assistance part of this service helps with independent housing. The Housing Transition Services part of this service assesses housing needs and develops an individualized housing support plan. The Housing Tenancy Sustaining Services part of this service helps people stay in the home they rent.
Transportation
CCI, Inc. may provide assistance with Transportation through reimbursement of public transportation, mileage, or ride share when a person is working and needs transportation to and from their job, or to access the community, and all other options have been explored. The Transportation service is offered by CCI as an Organized Healthcare Delivery System, and does not assume responsibility for transportation with outside parties such as rideshare, public transportation, or any other form of travel reimbursed for paid for by this service.
