Eligibility – Community Pathways Waiver (CPW)
People who eligible to receive services from the Center for Community Inclusion, Inc. (CCI) are 18yo or older, eligible for services through the Maryland Department of Health (MDH) Developmental Disabilities Administration (DDA), and have applied for or been approved for the Community Pathways Waiver.
A Medicaid waiver program, approved by the federal Centers for Medicare and Medicaid Services (CMS), allows states to make exceptions to the usual rules. This enables Medicaid and other services to be provided in different settings (home and community), tailored to individuals with different needs or varying income levels.
For those who qualify, this program offers services within the community as an alternative to receiving them in an institution. To be eligible, applicants must meet specific technical, medical, and financial criteria initially and on an ongoing basis. Individuals who are eligible for DDA services should work with their Coordinator of Community Services (CCS) to apply to the waiver.
The Community Pathways Waiver (CPW) helps participants to live more independently in their homes and communities. The program provides a variety of Meaningful Day, Support, and Residential Services that promote community living, including a self-directed service model and traditional, agency-based service model.
