If you are unhappy with your services or have concerns about your rights, you can report a grievance or concern at any time. We will listen, treat you fairly, and work to resolve the issue as quickly as possible.

How to Report a Concern

  • Speak in person 

  • Submit an anonymous grievance through the website

What Happens Next?

  1. We will listen to your concern. 

  2. We will talk with you about what happened. 

  3. We will work with you to find a fair solution. 

  4. We will respond as quickly as possible. 

  5. You will not be treated unfairly or punished for reporting a concern. 

If the issue cannot be resolved at the program level, it may be reviewed by:

  • The CCI Standing Committee 

  • The CCI Board of Directors 

  • CCS or DDA, if additional support is needed

You have the right to:

  • Be treated with dignity and respect 

  • Speak up about concerns 

  • Ask questions 

  • Receive fair treatment 

  • Be involved in decisions about your services
