CCI, Inc. takes all concerns and grievances seriously and will respond within 24 hours of receiving this form.

If you have provided your name and contact information, we will respond to acknowledge the concern and work with you for timely and fair resolution.

If this is an anonymous complaint, CCI, Inc will investigate the matter immediately. All concerns and grievances are presented to the CCI, Inc. Standing Committee for review.
