CCI, Inc. supports adults who are eligible for Developmental Disabilities Administration (DDA) funding through the Community Pathways waiver.
New to services? Apply for DDA services!
Once a person is eligible with DDA, they will work closely with their Coordinator of Community Services (CCS) to apply for the waiver.
CCI, Inc. is a DDA agency under the Traditional model of services. Applications to serve as a provider to an individual who self-directs services will also be considered. We are happy to serve Transitioning Youth, people new to services, and those transferring from other DDA providers, or adding new services.

You or your CCS may send referral information to ccireferrals1@gmail.com. Please send the most recent Person-Centered Plan, Health Risk Screening Tool and best point of contact for the individual applying for services.
Schedule a “meet and greet” with the CCI, Inc. team, learn about our services and a program that would be right for you. Your CCS is welcome to join the meet and greet too!
Once you’ve chosen to work with CCI, Inc. we will work closely with your team to develop a schedule, outcomes and goals and service plans to meet your needs. We will work with your CCS to get you started with services as soon as possible!
