DSP staff must ensure the health and safety of Individuals in services at all times. If the Individual’s health & safety are at immediate danger, DSP staff must contact 911 & only thereafter, DSP staff must follow all three steps listed below.
Step 1- TEXT 443-280-1893 to notify the CCI, Inc. Administrator that an incident has occurred – include the name of the Individual, brief description of incident and DSP staff name and number.
Step 2- CALL 443-280-1893 and speak to a CCI, Inc. Administrator to provide more details on the incident and to confirm the incident has been reported/received by CCI, Inc. Staff must speak with Administrator on call. Leaving voice messages is not considered notification.
Step 3- COMPLETE REPORT - DSP staff must report the incident through CCI, Inc. website portal at www.centerforcommunityinclusion.org within 24 hours after the Incident occurred.
An incident is an event or situation that may involve the infringement of personal rights, harm, or risk of harm to a participant including, but not limited to, the following:
Within 24 hours of the Incident, DSP staff will be directed to provide CCI, Inc. with a signed and dated IR document. CCI, Inc. will immediately suspend DSP staff involved in alleged Incidents involving Individual’s abuse, neglect, exploitation, theft, etc. until further investigation. DSP staff must cooperate fully with the CCI, Inc. during the investigation process.
